  • No. At Hercules Performance and Physical Therapy, we are considered an “out-of-network” provider and provide a “fee for service”. Check your out-of-network benefits and a superbill can be provided if requested for you to submit to your insurance company for reimbursement. We have an app -Reimbursify- that you can use to make the your process of submitting to your out of network insurance even easier! The only exception is Medicare - we are now accepting Medicare Part B.

  • Unfortunately, the current in-network model of physical therapy does not allow for the individual attention and detailed treatment that is provided by our Doctors of Physical Therapy at Hercules Performance and Physical Therapy. Here you will receive the high end, one-on-one services that you deserve.

  • Call the number on the back of your insurance card and use the worksheet below as a guide to ask questions. Also if you call us, we can look up you're out of network insurance benefits. However, it is your responsibility to verify your benefits and submit any out of network claims to your insurance company if needed.

  • Yes, we accept Medicare Part B (only) with a referral from your primary care or another approved healthcare provider. Not all services offered at Hercules are covered by Medicare, however, we will always be up front with you so you can make an educated decision on your care.

  • Your first visit will include a thorough full body evaluation, movement assessment, hands on treatment and education for you to gain better understanding of your body and how you move. During this time, our Doctors will get a better understanding of you as a person and your personal goals.

  • Follow up appointments will be based on findings during your initial evaluation and on an as-need basis. Because of our cash model, we are awarded the flexibility to cater to your individual needs and goals. Sessions will include combination of manual therapy body work, corrective exercises and strategies for home management.

  • NOT FOR 30 DAYS! In the state of Florida we have direct access for 30 days, meaning you can see a physical therapist directly up until day 30. At day 30, we would need a referral from your MD to continue physical therapy treatment. HOWEVER, we offer many services beside physical therapy that require no referral, including health/wellness, performance training, and recovery services. Our Doctor’s advanced training allow them to diagnose and treat based on impairments but also the knowledge to refer to a licensed medical practitioner if the condition lies outside their scope of physical therapy practice prior to the 30 day mark.

  • Please provide 24 hour notice if you need to cancel your appointment. For cancellations under 24 hours there will be a fee. No-show appointments will be charged the full price of the service.

    Extenuating circumstances and special situations will be reviewed on an individual basis.
